HCBS waivered 245 D services

Royal crown Homes is licensed to provide basic and Intensive 245D services in the state on MN.
All the 245D services are customized to the individual’s needs. Each individual will work with a trusted, qualified caregiver suited for their specific needs. All our care givers MUST have a cleared background check from DHS prior to providing any services.
RCH will work in unison with the client’s family, representative party and case manager to create a plan that meets the specific client needs. All services will be covered by a waivered service plan.
The sole purpose of our services is to intentionally strive to improve the well-being and independence of all the individual members entrusted to our care.
